Waiver and Release Packet
If you are participating in a Baruch exchange, or studying on the Spanish Language and Civilization course in Salamanca, you are required to submit a completed a Waiver and Release Packet. This packet contains:
- Study Abroad Participation Waiver and Release Agreement
- Certificate of Insurance Coverage
- Medical Report Form
- Emergency Contact Information
- Confidentiality Release Form
Some of these documents require notarization. Please ask the Study Abroad Office whether you need to submit a Waver and Release Packet.
