- Request to Pursue Dual Major or Change Major
- Students who wish to change their MBA major or pursue a dual major would use this form. Please note: do not use this form to request a change in degree objective (ie: MBA to MS, MS to MBA, or from one MS degree to another MS degree). To pursue a change in degree objective please consult the Office of Graduate Academic Services. Some restrictions may apply.
- Course Revision
- Use this form if you are requesting permission to take a course that does not appear as part of your program requirements.
- Core Courses Substitution (MBA)
- Use this form if you have completed 15 semester hours in a core area within the seven years prior to enrolling in the MBA program, and wish to substitute an advanced-level course in the same area for the core requirement.
- Transfer of Credit
- Read the instructions on the transfer credit application carefully and attach required documents with application. See also Graduate Bulletin: "General Academic Regulations/Transfer of Credit for Graduate Work and Permit Status."
- Make Up/Qualifying Exams
- Use this form if you are absent from a final exam or wish to take a qualifying exam to waive a preliminary course requirement. (Grades of ABS or INC must be resolved by the end of the final examination period of the subsequent semester, or they become grades of FAB or FINC, the equivalent of F.)
- Appeal to the Graduate Committee on Academic Standing
- Use this form if you wish to appeal a decision relating to your academic standing and the scholastic requirements of your program. See also Graduate Bulletin: "General Academic Regulations/Graduate Committees on Academic Standing."
- Extension of Time Limit for Degree
- Use this form to request an extension of time to complete your degree, not exceeding two semesters beyond the six-year time limit. Use the "appeal" form if you are requesting a longer extension. Students who interrupt their studies for one or more semesters, (i.e., are not enrolled at Zicklin or any other institution) have the original six years and up to two additional semesters to complete the degree before requiring a time extension. - Course Request Slip
- Office of Graduate Academic Services Waiting lists will be maintained for closed courses. Priority on waiting lists is determined primarily by the number of degree credits completed, to date, and the number of credits remaining for the degree. There is no guarantee that closed courses will re-open.
